Referral Coordination Program

What is the EHE Referral Coordination Program?

EHE is committed to making your EHE exam and care support services even better. As part of your EHE benefit, we offer the Referral Coordination Program to all patients who have completed their EHE exam within the current exam period. Our referral program is easy to navigate, convenient to use, and allows our patients to receive the best ongoing post-exam care available.

The referral program encourages greater use of preventive health care services by assisting our patients with scheduling referrals with healthcare providers both in and out of EHE’s provider networks. Based on the results of your most recent EHE exam, our referral team can assist you with finding physician referrals based on your preferences, health insurance information, and preferred geography. Upon patient request, the referral team can also coordinate the secure transfer of patient EHE exam records prior to your scheduled referral appointment(s).

There is no cost to use EHE’s referral program, however, you may be responsible for any co-payments, co-insurance, and/or deductibles to the referred health care provider(s) at the time of service.

How do I make a referral?

We always advise that you consult with your EHE physician that performed your EHE exam before making your referral(s). Additionally, you can submit the information from our website at EHEandMe.com to begin the referral review process and complete the scheduling of your referral appointment(s).

How the referral process works?

Step 1: To submit a request for a referral(s), go to EHEandMe.com > Health Records > Referrals > Current Referrals > Request a Referral
OR
Contact the EHE Referral Team by email Referral@EHEandMe.com or call 212-332-3657 during our normal business hours, Monday-Friday 8-5 p.m. EST.

Step 2: Once you have completed your submission, our referral team will review and process your request. Typically, requests will be processed within two business days.

Step 3: Your requests for finding physician that are approved will be available under “Referral Ready” on your Current Referrals screen. You may select the physician of your choice from the list and request for an appointment. Once your appointment request is approved, you will find information about your scheduled referral appointment(s) listed under “Confirmed Appt.” on your Current Referrals screen.
